REGISTRATION FORM: Consultix Summer Classes, 2010

   PREREQUISITES.  It is the responsibility of the student to ensure
     satisfaction of the course prerequisites, as listed on our Web Page.

   TERMS OF PAYMENT.  Payments to Consultix should be made by charge card
     or a check for U.S. funds drawn on a U.S. bank. Payment should be
     received within 30 days of registration or before the start of
     class, whichever comes first (although we can be flexible if
     necessary). In certain cases, we may accept a Purchase Order (PO)
     to confirm seating. To qualify for the discounted "Early-Bird
     Price", either full payment or an acceptable PO must be received
     by the Deadline shown below. A finance charge of .5% per week,
     compounded weekly, is charged for late payments. The act of
     registering indicates acceptance of these terms.

   CANCELLATIONS.  Prepaid tuition will be refunded for students who cancel
     by the Cancellation Deadline (the same day as the Early-Bird deadline),
     less 3% if paid by charge card. For payments arranged by PO, if
     cancellation occurs after the Deadline, payment in full will still be
     due.  Substitutions are allowed. In the event that we cancel a class,
     prepaid amounts may be refunded in full or applied to another class.
